Fans will need to provide a COVID pass, demonstrating that they have received two doses of the COVID-19 vaccine or have returned a negative test result in the 48 hours prior to the game.
This can be presented via the NHS app, a digital pass in your mobile phone wallet, a physical PDF copy or, if you are an EU citizen, as an EU Digital COVID Certificate.
If you are unable to provide any of the above passes, you may also present a negative COVID Test Result email to gain entry into the stadium.
Additionally, all fans aged 18 and over attending Sunday's match must complete a COVID status self-declaration.
We also ask that supporters respect one another and remind you that face coverings are now a legal requirement indoors and on the concourses, except when eating and drinking or sat at a table. We would also strongly recommend wearing face coverings while seated in the stands.
West Ham has confirmed that tickets will be distributed from Thursday 23 December for this fixture and only on completion of this declaration form. Failure to complete this form will lead to refusal into London Stadium.
